Everyone who enters into a contract with CIDA must complete the following declaration.

Purpose:	To comply with Government policy regarding the hiring of former public servants.
Return to:	CIDA representative you have been dealing with in the negotiation of the contract - must be received before the contract is signed by CIDA

Contractors and suppliers should only complete the following declaration when specifically requested in a CIDA proposal or bid.

Title:	Fair Price Declaration
Number:	CIDA 931
Details:	78 KB   1 page   8.5" x 11"
Purpose:	To substantiate that the prices quoted are competitive in current markets.
Return to:	CIDA representative designated in the proposal. Completed and signed declarations are to be submitted with accompanying tender, bid or proposal.

Title:	Total Funding Declaration
Number:	5715-00B
Details:	757 ko   1 page   8,5" x 11"
Purpose:	To be completed by the organization to declare all sources of funding received specifically for the project submitted to CIDA.
Return to:	CIDA representative you have been dealing with in the negotiation of a contribution agreement. A signed declaration must be received before the agreement is signed by CIDA.

Title:	Declaration regarding outstanding debts due to Her Majesty
Number:	5716-00B
Details:	717 ko   1 page   8,5" x 11"
Purpose:	To be completed by the organization to update a declaration should a new debt arise during the course of a contribution agreement.
Return to:	CIDA representative you have been dealing with regarding a contribution agreement.

Title:	Anti-Corruption Declaration
Number:	5717-00B
Details:	707 KB   1 page   8.5" x 11"
Purpose:	To be completed by whoever submits a proposal to CIDA in order to obtain financing in the form of a contribution or to indicate any change to a previous declaration.
Return to:	CIDA representative you have been dealing with regarding a project proposal. Completed and signed declarations must accompany the organization's submission to CIDA.
