Privacy Notice Statement

Provision of the information is on a voluntary basis. We will use the information for the purpose of establishing lists of pre-qualified candidates for evaluation or bid solicitation purposes.

The information will be held in CIDA 's Personal Information Bank number CIDA PPU 1525, and will be retained according to the retention and disposal schedule established for this bank.

Your personal information is protected under the provision of the Privacy Act. Under the Act, you have the right to request access and correction of your personal information, if erroneous or incomplete.

It should be noted that it is the consultant's responsibility to ensure that his/her profile is maintained, as it will be deleted if no update has been registered/recorded within the last 2 years.

Statement of Accuracy of Personal Information

I hereby certify that all of the information provided in this application is truthful and accurate. I understand that this statement has the same force and effect as if made under oath. I further undertake to keep this information up to date at all times and to report any change in this information as soon as possible. I agree to allow CIDA to verify this information at any time. I understand that if CIDA deems this information to be false, inaccurate, or incomplete, my file may be rejected or declared inactive.

If my file is declared inactive, I will be notified by registered mail. I will have up to one month from the receipt of this notice to remedy the situation. Failing this, my file will be rejected with no further notice or extension.
